A Contact Sport Notice
Plyometrics is a non-contact, highly physical sport. By signing below, you acknowledge that:
You understand the physical nature of the sport
You accept the inherent risks involved in plyometrics participation
Your child is physically able to participate in this program
Liability Waiver
By signing below, I release the program, coaches, staff, and organization from any liability for injury, accidents, or damages that may occur during participation. I understand that reasonable precautions will be taken for safety.
A full refund will be given if the cancellation of a session is made (2) weeks before the academy date. No refund will be given if cancellation is made within two (2) weeks of the session date. No refund will be given for no-shows or last-minute cancellations.
